The form must be filled in by the lead purchaser. All tickets in the transaction will be refunded. This will include Kids Fanzone and car parking tickets if purchased.
Refunds must be requested up to 7 days before the fixture is played, in line with our ticketing terms & conditions.
Please note, this form is only available to match-by-match ticket purchasers. Season Ticket Members should use the Ticket Exchange feature on your account for any home league matches you are unable to attend.
